I'd always suggest the least invasive method first (Chiropractor),
if that don't work there may be injections available, surgery should be
the last resort.

I was in a wheelchair with sciatica 41 days once. I pushed the chair,
bent over, sat in it, did things hopping around bent over. One night
was so miserable I swore this is the worst structural pain possible;
taking tylenol-3 (Codeine) I had ice bags or frozen vegetable bags on
the lumbar spine and on the affected spots on the affected leg with no
relief for hours one night.
Chiropractic didn't immediately help.
Finally I got an epidural steroid injection. This had me walking
straight in a few days, running an hour/day in a few weeks.
I always wanted to try regular chiropractic adjustments, but the cost
was prohibitive. In town here (San Diego/El Cajon, CA) there is a
chiropractic group that offers unlimited adjustments for $45/mo. in a
two-year plan. I've been ok since, it is very good maintenance. That is
the strength of alternative medicine. The straight docs are better at
surgery and emergencies.
